Abstract Responsiveness to the transfusion requirements of clinical transplantation patients is essential for quality patient care and optimal graft survival. Transfusion requirements range from zero through type and screen to hundreds of units of blood components depending upon the type of graft. Abstract The dramatic increase in the number and type of transplant procedures performed has required support from the therapeutic drug monitoring laboratory. Of the immunosuppressive agents used, only cyclosporine has been shown to require routine monitoring for individualization of therapy. Cyclosporine monitoring has a number of controversial aspects, including the sample matrix and the analytical method employed. Transplant patients are also sometimes subject to impressive polypharmacy as a result of their severe illness, a factor which necessitates careful attention to monitoring methods.
