Anders Pathologisches Institut der Universität Tübingen Tübingen Deutschland Summary We have determined quantitatively the mesangial volume and the number of glomerular cells in 30 kidneys of diabetics by means of semi-thin sections. The results were explored with respect to the insulin-binding capacity of the serum, the cholesterol level, chronic renal failure and the therapy. We detected a considerable increase in the mesangial volume, up to twice that of the standard. We also observed a true proliferation of the mesangial and endothelial cells. The decrease in epithelial cells was caused by the adaptation to the simplified glomerular structure. The proliferation was on an average 47% compared with normal controls. The increase of the mesangial volume and the mesangial cells is characterized by a positive correlation to the titre of insulin antibodies Correlations of mesangial volume and of the number of mesangial cells to the level of serum cholesterol could not be fixed statistically. After long-term therapy with insulin we found higher mesangial volumes than after sporadic insulin therapy or application of oral antidiabetic agents. We see in the diabetic glomerulosclerosis a glomerular disease characterized by an increase of the mesangium and by a true cell proliferation. In the description of the morphometric features it does not differ from an intercapillary proliferative glomerulonephritis. It is possible that these changes are caused by an immune-process in the mesangium produced by insulin-anti-insulin complexes.
