It is well established that mental health services in the UK have been slow to adopt evidence-based practice (EBP) and this applies in particular to cognitive behavioural (CB) approaches ( Kavanagh et al 1993 , Fadden 1997 , Price 1999 , Tarrier et al 1999 , Brooker et al 2003 ). However, the literature is flawed. First, authors have tended merely to redescribe the problem rather than analysing it. Second, others have offered rational solutions, such as calling for service reconfigurations and cascading training. They have ignored important and fundamental features of organisations that undermine attempts to implement CB approaches.
