Since the staff-led scheme was introduced, there have been no cases of pressure ulcers for more than 18 months on a number of wards at Abertawe Bro Morgannwg University Health Board. The SKIN Bundle reminder system, which received an NHS Wales Award 2011 recently, involves checking the surface of the skin, keeping the patient moving and assessing continence and nutrition status. Putting the system into practice are junior nurse Zoe Edwards and lead nurse in long-term care Beverley Williams. Assistant director of nursing Nicola Williams said: ‘We are determined to make pressure ulcers a thing of the past.’ Download a powerpoint presentation http://tiny.cc/SKIN_Bundle
