Luteinising hormone pulsatility in a spontaneous conception cycle

LYNCH Department of Clinical Endocrinology, Birmingham and Midland Hospital for Women The secretory dynamics of luteinising hormone (LH) in women with ovulatory and anovulatory menstrual cycles have been reported previously by our group (Clayton et al., 1987). Information on L H pulsatility in a spontaneous conception cycle has not been documented before. Case report A 30 year old woman presented with primary infertility for 36 months. She had regular 28-35 days cycles. Endogenous gonadotrophin releasing hormone secretion was studied by measurement of the pituitary LH response with characterisation of pulsatile LH release at 10min intervals over 8 hours o n two occasions during the course of follicular development and once during the M e a l phase of the same cycle. Luteinising hormone assay variability, peak detection and statistical analysis corresponded to Clayton et al. (1987) and Chapman et a/. (1987). Ovulation was documented with serial ultrasound ovarian tracking and luteal phase progesterone. Pregnancy was confirmed in the same cycle by hCG
