Curr Opinion Hematology 13:447-451, 2006 This concise review with an annotated bibliography should be of interest to those readers of Transfusion Medicine Reviews who are struggling with requests for fresh frozen plasma (FFP) to bprepareQ a patient for an invasive procedure. The authors review the published evidence, or in this case the lack of published evidence, that preprocedure FFP improves outcomes for patients with mild to moderate prolongation of laboratory clotting tests. Included in the article are several practical reports including one that shows that bleeding outcomes did not correlate with preprocedure International Normalized Ratio values in a group of patients undergoing placement of an intracranial bolt for pressure monitoring. This latter article (J Trauma 2004;57:1173-1177) might be of particular value to those readers who are working with neurosurgical colleagues. In our program, the topic of intracranial bolt placement stirs a lot of controversy. There is disagreement over the indications for bold placement and disagreement over the extent to which monitoring improves patient outcomes.
