Lee 2 1 Department of Ophthalmology, 2 Department of Pathology, National University Hospital, Singapore Purpose: To report three cases where biopsy of the levator superioris muscle resulted in interesting histopathologic findings. Methods: Three Cases that presented with unilateral moderate to severe ptosis with fair to good levator function are described. All three patients underwent levator palpebrae superioris muscle resection. During the course of management, a levator muscle biopsy was deemed necessary for all three patients and sent for histopathologic studies, with special stains and in one case, for electron microscopy. Results: The first case presented with a 3 year history of left upperlid ptosis, biopsy results showed amyloidosis as evidenced by special stains namely Congo red,immunostainand stain for Amyloid-P. The second case consulted for droopy left eyelid of many years, biopsy showed the presence of abnormal mitochondria. Oxidative enzyme stain and electron microscopy done suggested a mitochondrial myopathy. The third case had right upper lid ptosis since childhood, and underwent two corrective surgeries but ptosis remained unchanged, intraoperatively a biopsy was done which revealed a traumatic neuroma.
